What do I need to do to go skydiving?

I am a Type 1 Diabetic and I live in Phoenix, Arizona. I want to go on the tandem skydive. I am only 15 years old, and I wanted to go skydiving on my 16th birthday. I'm appreciative of any info. Thank You
