House-training somewhat unsuccessful. Help!?

My dachshund puppy is now approaching 6 months old. We have not used potty pads in 3 months. For several weeks, she had a perfect record and only went outside on a strict schedule of being taken for a walk outside 4-5 times a day. Then it all fell apart and she started going in the house again. We have a "time out" kennel, which is small and unfurnished, and she has her crate with her bed - when she goes potty in the house, she goes in time out so she doesn't confuse her bed with punishment. We are very consistent, and we give her treats and sing her praises when she goes potty outside. When she goes potty in the house, she immediately dotes at my feet and looks miserable. She knows she's done wrong, but she refuses to ask to go outside - in between her regular walks. The one or two times she did ask, I was sure to respond immediately. Now she has been peeing on the furniture - our bed and couch, which she has been temporarily banished from. What do I do next?
